The Common Carrier Accidental Death and Dismemberment benefits with 24 Hour Extension and Car Rental Accidental Death and Dismemberment benefits are underwritten by Allianz Global Risks US Insurance Company ("Allianz") under Group Policy No. FC310000-C for a BMO Corporate Card account; or Group Policy No. FC310000-D for Diners Club Corporate Card account, Diners Club Event Card account and Diners Club Travel Account issued to Bank of Montreal (the "Policyholder", "BMO").
